To become a new patient, we will need a physician referral from your PCP or the doctor that is treating your pain.
● Please have the office fax us your records (office visit notes, imaging, demographics including insurance information). Fax to 410-641-1746.
● Once we get everything we need, we will verify benefits and eligibility with your insurance company. The verification process usually takes 5-7 business days.
